Tous les patients cardiaques sous traitement \u00e0 Maastricht UMC (MUMC+) peuvent d\u00e9sormais mesurer leur rythme cardiaque \u00e0 domicile. En pratique, les malades transmettent directement leurs donn\u00e9es via une application d\u00e9velopp\u00e9e par FibriCheck. L'assureur VGZ et MUMC se sont mis d'accord sur le remboursement standard de la t\u00e9l\u00e9surveillance des patients cardiaques. VGZ rembourse les soins num\u00e9riques, faisant de la t\u00e9l\u00e9surveillance un \u00e9l\u00e9ment permanent des soins pour traiter les patients souffrant d'arythmie cardiaque.<\/strong><\/p>\n

Un d\u00e9veloppement acc\u00e9l\u00e9r\u00e9 par la COVID-19<\/h2>\n

Sous la pression de l'\u00e9pid\u00e9mie actuelle, l'h\u00f4pital de Maastricht MUMC+ a d\u00e9velopp\u00e9 une nouvelle infrastructure mHealth. Elle a \u00e9t\u00e9 mise en place en avril 2020 sous le nom de TeleCheck-AF (AF signifie fibrillation auriculaire). L\u2019objectif est de suivre \u00e0 distance les patients atteints de fibrillation auriculaire. Pour y parvenir, le suivi est fait via l\u2019analyse de la fr\u00e9quence et le rythme cardiaque avec une application et des consultations t\u00e9l\u00e9phoniques.<\/p>\n

Dominik Linz, cardiologue \u00e0 Maastricht UMC+ explique les enjeux de TeleCheck-AF. \u00ab Le d\u00e9veloppement de l'application de TeleCheck-AF est le plus grand projet dans le domaine des soins num\u00e9riques depuis la crise du coronavirus en Europe. \u00c0 l'heure actuelle, 41 centres - avec plus de 4 000 patients - utilisent l'application Fibricheck. Les soins \u00e0 distance ne sont donc plus une exp\u00e9rience, mais sont vraiment devenus une partie de notre travail. Il ne peut en \u00eatre autrement. Je constate d\u2019ailleurs que les patients en ont besoin tous les jours. J'esp\u00e8re que d'autres h\u00f4pitaux aux Pays-Bas et dans toute l'Europe suivront bient\u00f4t.<\/em> \u00bb<\/p>\n

Lancement de FibriCheck au d\u00e9but de la pand\u00e9mie<\/h2>\n

Initialement, FibriCheck \u00e9tait gratuit pour les centres m\u00e9dicaux.<\/a> L\u2019outil servait d\u00e9j\u00e0 \u00e0 garantir les soins aux patients atteints de rythme cardiaque, explique Bieke van Gorp de FibriCheck. \u00ab Les patients ont utilis\u00e9 FibriCheck dans les 7 jours pr\u00e9c\u00e9dant la t\u00e9l\u00e9consultation pour collecter des donn\u00e9es, en moyenne 3 fois par jour. Les m\u00e9decins avaient acc\u00e8s aux informations et recevaient \u00e9galement un rapport tout \u00e0 fait clair avant la t\u00e9l\u00e9consultation. Ces donn\u00e9es objectives servent \u00e9galement lors de la t\u00e9l\u00e9consultation. Entre-temps, nous avons appris que cette approche fonctionne \u00e9galement en dehors de la pand\u00e9mie. En effet, de nombreux patients peuvent \u00eatre parfaitement suivis \u00e0 domicile. Cela \u00e9vite les visites physiques chez le m\u00e9decin et les diagnostics suppl\u00e9mentaires (co\u00fbteux) qui ne sont pas pour beaucoup n\u00e9cessaires.<\/em> \u00bb<\/p>\n

de l'exp\u00e9rimentation \u00e0 l'offre standard<\/h2>\n

Avec la nouvelle prestation facultative de l'autorit\u00e9 n\u00e9erlandaise des soins de sant\u00e9 - l'organisme qui s'occupe des remboursements - il existe d\u00e9sormais un remboursement structurel pour cette forme de soins. Ainsi elle peut passer de l'offre exp\u00e9rimentale \u00e0 l'offre standard. Au d\u00e9part, il existait un accord local entre l'h\u00f4pital et l'assurance-maladie concernant le remboursement de ces soins. D\u00e9sormais cet accord s'applique \u00e9galement aux autres h\u00f4pitaux. Ils peuvent conclure des accords avec le ou les assureurs-maladie concernant cette forme de soins num\u00e9riques.<\/p>\n

Gr\u00e2ce au t\u00e9l\u00e9contr\u00f4le de la fibrillation auriculaire (FA), les patients cardiaques peuvent facilement mesurer leur rythme cardiaque via une application sur leur smartphone. Ces mesures peuvent ensuite \u00eatre \u00e9valu\u00e9es \u00e0 distance par un cardiologue afin d'aboutir \u00e0 des conseils de traitement. La t\u00e9l\u00e9surveillance est une alternative valid\u00e9e aussi pr\u00e9cise que le suivi qui se fait \u00e0 l'h\u00f4pital pour ces patients, selon la NZa.<\/p>\n

Une aide pr\u00e9cieuse pour des millions de patients<\/h2>\n

\u00c0 l'\u00e9chelle nationale, environ 142 000 patients consultent un cardiologue chaque ann\u00e9e et sont \u00e9ligibles \u00e0 l'utilisation de TeleCheck-AF. Si tous les autres h\u00f4pitaux utilisent TeleCheck-AF, une \u00e9conomie d'environ 24 millions d'euros en frais de sant\u00e9 est possible, ont calcul\u00e9 le MUMC+ et la VGZ.<\/p>\n

Depuis l'\u00e9pid\u00e9mie, environ 1 400 patients du MUMC+ ont \u00e9t\u00e9 suivis avec TeleCheck-AF. Selon l'h\u00f4pital, le nombre d'\u00e9lectrocardiogrammes et d'examens Holter par trajectoire de soins a ainsi consid\u00e9rablement diminu\u00e9. Les t\u00e9l\u00e9consultations ont remplac\u00e9 les rendez-vous physiques \u00e0 l'h\u00f4pital.<\/a> Un b\u00e9n\u00e9fice pour tous puisque les patients et leurs familles \u00e9conomisent du temps et de l'argent. Cela signifie aussi une plus grande commodit\u00e9. Les malades n'ont pas \u00e0 se d\u00e9placer \u00e0 l'h\u00f4pital ou en tout cas moins souvent.
