L'application permet aux patients en dialyse p\u00e9riton\u00e9ale (PD) de mieux se surveiller.<\/strong><\/p>\n<p>L'initiative de ce nouvel outil revient \u00e0 Kim Vedder, une infirmi\u00e8re sp\u00e9cialis\u00e9e travaillant au service de dialyse de l'h\u00f4pital d'Alkmaar. Elle a con\u00e7u l'application dans le cadre du <em>NierstichtingChallenge<\/em>, qui met en avant les id\u00e9es novatrices am\u00e9liorant les soins et le suivi des patients atteints de maladies r\u00e9nales. En tout, plus de 20 projets du <em>Kidney FoundationChallenge<\/em> ont d\u00e9j\u00e0 \u00e9t\u00e9 lanc\u00e9s depuis l'ann\u00e9e derni\u00e8re.<\/p>\n<h3>Num\u00e9risation des constances \u00e0 v\u00e9rifier<\/h3>\n<p>Kim Vedder suit le cours <em>Advanced Health Informatics Practice<\/em>, pr\u00e9cise le groupe hospitalier. \u00ab <em>En concertation avec les n\u00e9phrologues de Noodwest, elle a \u00e9tudi\u00e9 une application permettant aux patients atteints de PD de saisir num\u00e9riquement leurs mesures. Ces donn\u00e9es renseign\u00e9es manuellement dans l\u2019EPD sont ainsi \u00e0 tous moments visibles par les th\u00e9rapeutes.\u00bb, se r\u00e9jouit Kim Vedder. <\/em>L\u2019infirmi\u00e8re a soumis son projet lors du <em>NierstichtingChallenge<\/em> et a utilis\u00e9 la subvention obtenue pour d\u00e9velopper son id\u00e9e aux personnes atteintes de la maladie de Parkinson.<\/p>\n<p>Gr\u00e2ce \u00e0 ce nouvel outil de sant\u00e9 en ligne, les patients peuvent visualiser plus facilement leurs consommations de boissons. \u00ab <em>Si les patients comprennent mieux comment la r\u00e9gulation de leur liquide fonctionne, ils pourront \u00e9ventuellement ajuster leurs traitements de mani\u00e8re ind\u00e9pendante en consultation avec le n\u00e9phrologue. Cela leur donne une certaine ind\u00e9pendance, ce qui leur fait souvent du bien. Les patients en dialyse saisissent directement leurs mesures dans My Noordwest. Ces derni\u00e8res, qui sont consultables dans l'EPD, permettent ainsi aux l'infirmi\u00e8res de v\u00e9rifier \u00e0 distance si tout se passe bien. \u00bb<\/em><\/p>\n<p>Dans le portail des patients du groupe hospitalier, une section sp\u00e9cifique leur est d\u00e9di\u00e9e. En plus de ces donn\u00e9es, les personnes remplissent \u00e9galement un court questionnaire sur leurs \u00e9ventuelles douleurs physiques. Une autre page est disponible dans l'EPD, mais cette fois, uniquement pour les praticiens. Cette derni\u00e8re regroupe les donn\u00e9es m\u00e9dicales des patients. Ce qui favorise grandement la rapidit\u00e9 du suivi par le personnel m\u00e9dical.<\/p>\n<h3>Un contr\u00f4le permanent<\/h3>\n<p>En utilisant ce nouvel outil, les patients sont plus alertes. Si jamais ils boivent un peu plus que d\u2019habitude, par exemple apr\u00e8s un anniversaire ou lors d'une journ\u00e9e tr\u00e8s chaude, ils peuvent le constater imm\u00e9diatement.
