La Plate-forme flamande des patients et l'association de m\u00e9decins g\u00e9n\u00e9ralistes, Domus Medica, ont donc uni leurs forces pour lancer la campagne \u00ab <em>Quality &amp; Good Care<\/em> \u00bb. L'objectif est d'encourager les citoyens \u00e0 maintenir\u00a0 \u00e0 jour leurs ant\u00e9c\u00e9dents m\u00e9dicaux.<\/strong><\/p>\n<p>Un historique \u00e9lectronique permet aux professionnels de soins de sant\u00e9 de consulter en une lecture rapide les informations les plus importantes d\u2019un dossier m\u00e9dical. Ils peuvent ainsi conna\u00eetre rapidement les allergies potentielles, les ant\u00e9c\u00e9dents m\u00e9dicaux et les probl\u00e8mes de sant\u00e9 actuels. Ces renseignements sont particuli\u00e8rement importants lorsqu\u2019il s\u2019agit d\u2019une urgence chez un m\u00e9decin g\u00e9n\u00e9raliste ou en cas d\u2019inconscience d\u2019un patient arriv\u00e9 \u00e0 l\u2019h\u00f4pital.<\/p>\n<h3><b>Seulement 3 millions de fiches compl\u00e9t\u00e9es<\/b><\/h3>\n<p>Pourtant, tous les citoyens de Flandre ne disposent pas d'un historique m\u00e9dical d\u00fbment compl\u00e9t\u00e9. D\u2019apr\u00e8s Vitalink, la banque de donn\u00e9es de sant\u00e9 flamande issues des soins primaires, seulement 3 millions des Flamands (3 011 154) disposent d'un tel fichier r\u00e9capitulatif. Ilse Weeghmans, Directrice de la Plate-forme flamande des patients, s'en inqui\u00e8te. \u00ab<em>Une grande partie de la population n\u2019est pas au courant d\u2019une telle d\u00e9marche ou n\u2019a toujours pas renseign\u00e9 leur fiche. En outre, les informations sont parfois manquantes ou non actualis\u00e9es. Cela peut \u00eatre pr\u00e9judiciable \u00e0 la continuit\u00e9 des soins. \u00bb<\/em><\/p>\n<p>Selon le Docteur et Pr\u00e9sident Domus Medica, Roel Van Giel, le m\u00e9decin g\u00e9n\u00e9raliste doit se charger de veiller \u00e0 ce que toutes les fiches de ses patients soient en ordre. En effet, c\u2019est lui qui d\u00e9tient le dossier initial, le Global Medical Record (GMD). Il doit ainsi v\u00e9rifier que les informations de base sont compl\u00e8tes et \u00e0 jour. \u00ab <em>Ces m\u00e9decins de proximit\u00e9s jouent un r\u00f4le important puisqu\u2019ils disposent d\u2019une large vision de l\u2019\u00e9tat de sant\u00e9 d\u2019une personne. Ils compl\u00e8tent ainsi les informations que doivent fournir les patients.\u00bb<\/em><\/p>\n<h3><b>Instaurer \u00e0 nouveau un dialogue de confiance<\/b><\/h3>\n<p>Avec la campagne <em>\u00ab Quality &amp; Good Care \u00bb<\/em> (Qualit\u00e9 et bons soins), la Plate-forme flamande des patients et Domus Medica cherchent \u00e0 favoriser le dialogue entre les patients et les m\u00e9decins g\u00e9n\u00e9ralistes. En m\u00eame temps, c'est \u00e9galement un rappel \u00e0 tous les citoyens de Flandre. \u00ab <em>Nous souhaitons faire passer le message suivant : consultez votre docteur de famille et remplissez avec lui votre fiche de sant\u00e9<\/em> \u00bb, d\u00e9clare Ilse Weeghmans.<\/p>\n<p>Une brochure a \u00e9t\u00e9 \u00e9labor\u00e9e afin de sensibiliser le maximum de personnes. Cette derni\u00e8re a d\u2019ailleurs \u00e9t\u00e9 con\u00e7ue par la Vlaams Pati\u00ebntenplatform, l'organisation qui chapeaute plus de 115 associations flamandes de patients atteints d'une maladie chronique, en collaboration avec La LUSS (La Ligue des Usagers des Services de Sant\u00e9 asbl) et Domus Medica.
